For the registration form, download, print & fill out form. You can also pick up a hard copy or phone or text Nichole Charles at 420-9153 and she can email you a copy.
Scan and email completed form to [email protected] or fax to 425-3031 or drop off hard copy with Nichole at JRMCC.
